Carry
out a risk assessment for the entry and making inspection.

Identify
the potential hazards and eliminate them.
→
The
tank to be empty & stripped.
→
Secure
the space against the ingress of dangerous substances by blanking off
pipe lines or other openings and closing & tieding up valves.
→
Open
at least 2 manholes.
→
Thoroughly
ventilate the tank.
→
Keep
the space illuminated and well ventilated throughout.
→
Check
the tank atmosphere for oxygen. Check the sample in different levels
of the tank. Use sampling tube, aspirator & oxygen analyser.
Ensure oxygen is at least 20.9% by volume.
Use
combustible gas detector for flammable gas (not more than 1% LEL).
→
Select
the person in charge for the operation.
→
Nominate
the person for entry and brief them accordingly.
→
Post
notice in E/room and cargo control room.
→
Inform
Master & watch officer.
→
Issue
a ‘’Permit to work’’.
→
Carry
out a tool box meeting.
→
At
least one person to be present at the access at all times.
→
Wear
appropriate PPE.
→
Establish
& check communication.
→
Establish
reporting intervals & procedures for recording all reporting with
times.
→
Make
entry –BA sets with spare bottles, escape sets, safety harness,
flash light, stretcher, heaving line and life line etc.
→
Complete
the inspection before the permit expires. Otherwise re-assess and
make new permit.
