Any type of occurrence on board to be entered in narrative section.



Entry into Narrative Section :

changes of Master

discharge of seafarers, i.e. when they sign off the list of crew (note there is no need to make an entry when crew members sign on to the list of crew, this is covered above).

promotions and demotions

wages disputes

desertions

complaints

disciplinary matters

criminal convictions during a voyage

accidents

casualties

illness

deaths

details of crew left behind

appointments of safety officers, representatives and committees.

meetings of safety committees.

the annexing of other documents to the OLB

closing of OLB.



Note : OLB last for 6 months from the time first person signs it.



Entries by Master in person :

o Documents handed over by Master.

o Departures and Arrivals.

o Reasons for not going to assistance.

o Crew left behind.

o Officer’s misconduct.

o Prosecution of crew advised.

o Births.

o Deaths.

o Deceased seaman’s property.
